"Steakhouse Syndrome" pada Pasien Dewasa di RSUP Prof. dr. I.G.N.G. Ngoerah, Denpasar

Sumbatan benda asing di esofagus merupakan kondisi kegawatdaruratan yang umum dan menempati urutan ketiga setelah perdarahan saluran cerna atas dan bawah. Kondisi ini dapat mengancam nyawa, tergantung pada jenis zat
yang ditelan, lokasi sumbatan, kondisi pasien, dan jangka waktu hingga terapi diberikan. Insiden tahunannya dilaporkan sebesar 13 per 100.000 populasi umum. Pada laporan ini, disajikan kasus seorang pasien perempuan berusia 30 tahun
yang datang dengan keluhan sulit menelan sejak dua hari sebelum masuk rumah sakit setelah mengonsumsi steak sapi. Keluhan disertai rasa nyeri dan terbakar di dada. Pada pemeriksaan didapatkan patensi jalan napas masih baik dan tidak ditemukan komplikasi seperti hematemesis maupun bunyi napas abnormal. Hasil pemeriksaan endoskopi gastrointestinal bagian atas menunjukkan adanya sumbatan makanan (massa steak yang tidak cukup dikunyah) pada esofagus, berlokasi 20 cm dari gigi, disertai erosi pada mukosa di sekitarnya. Lokasi ini sesuai dengan penelitian sebelumnya di mana pada orang dewasa, sekitar 68% dari obstruksi terjadi di daerah esofagus distal, yaitu di antara arkus aorta dan lower esophageal sphincter (LES). Pada kasus ini tidak diberikan obat-obatan untuk mengendurkan otot polos LES. Tindakan pendorongan
daging dari esofagus ke arah gaster dilakukan dengan bantuan endoskopi. Sumbatan berhasil disingkirkan dan keluhan pasien dapat teratasi. Dapat disimpulkan gejala pada pasien ini disebabkan oleh massa akut dari steak, yang dikenal sebagai “steakhouse syndrome”.
