Abstrak

Latar belakang: Acne vulgaris (AV) merupakan penyakit inflamasi kronis pada unit pilosebasea dengan manifestasi klinis berupa komedo terbuka dan tertutup, papul, pustul dan nodul. Terapi AV umumnya melibatkan lebih dari satu obat dikarenakan patogenesis AV yang multifaktorial. Tujuan: Penelitian ini bertujuan untuk mengetahui pengaruh pemberian astaxanthin oral 4 mg sebagai terapi adjuvan AV terhadap penurunan kadar serum tumor necrosis factor alpha (TNF-α) yang merupakan salah satu penanda inflamasi. Metode: Penelitian ini merupakan penelitian double-blind randomized controlled trial. Sampel berjumlah 34 pasien AV derajat ringan dan sedang (skor Global Acne Grading System/GAGS 1-30) yang diobati dengan kombinasi tretinoin 0,025% dan clindamycin phosphate 1,2%. Pada kelompok perlakuan ditambahkan terapi dengan astaxanthin sementara pada kelompok kontrol diberikan plasebo berisi laktulosa selama 4 minggu. Hasil pretest dan posttest pada kedua kelompok dianalisis secara statistik menggunakan uji nonparametrik. Hasil: Penurunan kadar serum TNF-α terjadi pada kedua kelompok, namun perbedaan yang bermakna dengan baseline hanya dijumpai pada kelompok pelakuan (p=0,015). Tidak terdapat perbedaan bermakna pada perbandingan penurunan kadar serum TNF-α pada kelompok perlakuan dan kontrol (p=0,459). Kesimpulan: Meskipun secara keseluruhan tidak menunjukkan perbedaan yang bermakna, namun pemberian astaxanthin dapat menurunkan kadar serum TNF-α secara bermakna terhadap baseline pada pasien acne vulgaris.
